The Heart Failure nurse works as an autonomous practitioner in managing patients with chronic heart failure. The main aims of the job are to: optimize evidence based pharmacological and non pharmacological treatments and to provide education to patients and carers. Reduce hospital readmissions through inpatient engagement and support facilitate improved and quicker safe discharge planning. Improve quality of life facilitate and palliative care. You will provide education to nursing colleagues and other professions at both local and national level. It is essential that you are currently a Registered Nurse with the NMC and that you have substantial post registration experience with relevant experience in cardiology. You should have Experience in heart failure management, clinical assessment skills and pharmacological knowledge. The shift pattern for this post is Monday – Friday 9.00am – 5.00pm.
