The Goldenhill Older Adults Community Mental Health Team are looking to recruit a band 6 Community Mental Health Nurse to their team. You will be responsible for managing a caseload providing comprehensive and holistic assessment, planning, implementation and evaluation of health and social care needs for older adults with a variety of often complex mental health and social care issues. As an integral member of the multi-disciplinary team you will work in partnership demonstrating compassionate caring behaviours to fulfill the key functions of the OACMHT. You will be responsible for proactively promoting partnership working with patients, carers and other professionals/ agencies in line with legislative frameworks, agreed standards and clinical policies. This involves working in a variety of home/ near to home community environments. You are required to undertake duties without direct supervision as well as supporting and supervising work undertaken by registered and unregistered nursing staff. Including providing caseload line management supervision and clinical supervision and undertaking line management responsibilities. It is essential that you are currently a Registered Mental Health Nurse with the NMC and that you have completion of evidence based psychological therapies/ psychological approaches to care formal programme of study. The shift pattern for this post is Monday – Friday 9.00am – 5.00pm.
