Annex 21 may be considered for Trainees.  


These posts are based in Hamilton.


The successful candidates will work autonomously across Primary, Community and Acute Services during the In and Out of Hours period covering 7 days including public holidays.  They will be responsible for clinical decision making including assessment and treatment of patients in across the Primary, Community and Acute settings.


They will work closely with the multi-disciplinary team (MDT)/medical staff including OOH Hub and Acute Services and communicate on a daily basis about any patient they have seen overnight/at weekends and who needs to be followed up by the GP/Acute Services during normal working hours.  They will provide assessment,  clinical care and management of both stable and acutely unwell  patient and his/her family making complex, autonomous decisions using advanced skills and knowledge and will have overall responsibility for the assessment of care needs and the delivery of advanced care.  Medical Telehealth/telephone support will be available when required from an identified source.


The successful candidates must be a Registered Nurse, AHP or Pharmacist with valid NMC/HCPC/ GPHC registration.  They will be educated to, working towards or operating at Masters level. They will possess a Postgraduate Diploma in Advanced clinical practice and expertise within speciality/area of practice and have comprehensive knowledge of clinical guidelines and standards.  Advanced specialist knowledge and experience of relevant conditions, pathology, policies and procedures associated with the speciality/area of practice are essential.  The successful candidates will demonstrate evidence of application of advanced practice skills in speciality and applying clinical judgement. The post also requires evidence of further developed knowledge and experience in management and/or leadership and further education/evidence of CPD.
