In the absence of the Service Manager the post holder will be responsible for the operational management of Discharge Services.
The successful candidate will take the operational lead in discharge of patients as part of a multi-disciplinary/ multi-agency team and will as part of the multi-disciplinary team have responsibility for expert assessment of care needs and the development, implementation, and evaluation of care ensuring the delivery of a high quality admission assessment/discharge planning, of all patients and their carers, particularly those with highly complex needs.They will be an autonomous practitioner who manages a highly complex caseload, maintaining ongoing responsibility in conjunction with the mulit-disciplinary team, for effective, patient centred discharge planning throughout the hospital. Requiring the development of specialised care plans to ensure effective discharge.The Integrated Discharge Co-ordinator will be responsible for leading and contributing to service developments and innovation within the discharge service and will be responsible for evaluating, implementing and monitoring discharge planning standards, giving advice and education as appropriate within the acute hospital, community setting, local authorities and the third sector.
Applying candidates must be a Registered Nurse AHP with valid NMC HCPC Registration, educated to/working towards/operating at Masters Level, havepost graduate expertise within speciality / area of practice that demonstrates the required breadth of knowledge required to lead safety, effectively and efficiently, comprehensive knowledge of clinical guidelines and standards within the speciality, advanced Specialist knowledge and experience of relevant conditions, pathology, policies and procedures associated with the speciality/area of practice ensuring that the level of expertise can be utilised to deliver leadership within speciality, evidence of application of advanced practice skills in speciality and applying clinical judgement, evidence of further developed knowledge and experience in management and/or leadership, further education/evidence of Continuous Professional Development in area of speciality including study days/courses/post graduate qualification, the post holder will possess excellent team-working/leadership skills and have the ability to motivate and influence others and work using own initiative, excellent listening, communication and interpersonal skills and effective time management skills and a working knowledge of basic information technology.
