SALARIED GENERAL PRACTITIONER - FRAILTY

Midlothian Health and Social Care Partnership has an ambitious vision to transform how we care for people living with frailty. We are testing new approaches which focus on proactive, coordinated care through multidisciplinary teams that are anchored to General Practice. Our model will evolve as we test and learn what works for frail people and as we explore options for how to care for people with frailty in care homes and intermediate care services.
We are looking to recruit an experienced General Practitioner with a passion for improving outcomes for people with frailty by working in integrated teams.
The post can be full or part-time and will be based in Midlothian. The role will require working in different care settings including general practices, care homes and Midlothian Community Hospital. Home visiting is likely to be a significant part of the role.
The post is initially fixed term for 24 months.
