Within a multi-disciplinary team you will have responsibility for assessing, planning, implementing and evaluating the care needs of older adults with Mental Health problems. Emphasis will be on care and treatment using validated psychological approaches and working as an integrated service to meet complex needs of this client group.   You will be responsible for providing supervision to Band 5 nurses, untrained staff and nursing students. Experience in Older adults Mental Health is desirable.  You will provide a service to a defined geographical area for patients who are experiencing mental health problems and also have a key role in promoting and developing the Community Mental Health Team service development.   You will provide high quality, culturally sensitive and effective specialist nursing to older people with mental illness living in the community by working in partnership with service users, care providers and other agencies.  You will provide triage, assessment, ongoing interventions and evaluation of these, participate in discharge planning processes and facilitate the coordination of care working collaboratively with patients using a biopsychosocial model within community OPMH services.  You will provide expert clinical advice, practical support and specialist education to other professional groups/staff regarding the mental health needs of the older adult population.  You will identify patients with complex needs on their caseload.  Through managing your own case load and in supervision of junior staff you will explore treatment options, stepping care up as needed and when required provide crisis interventions to offer specialist care in the community avoiding where possible the need for psychiatric inpatient stay.  You will require excellent communication skills in order to develop good relationships with other agencies/teams both primary & secondary care providers and voluntary agencies in order to enhance integrated care provision within the neighbourhood model.  It is essential that you are currently a Registered Nurse with the NMC and that you have relevant community experience at a Band 5 or similar experience.  The shift pattern for this post is Monday – Friday 9.00am – 5.00pm (day off still to be negotiated)
