An exciting post is currently available in Fife’s Community Services as part of a developing Community Frailty team.
We are a group of General Practitioners (GP) dedicated to challenging and changing the way in which we provide care to people with frailty. If you choose to join our team you will be working at the heart of health and social care integration, working across the primary and secondary care interface. This is a unique opportunity designed to enhance the role of the GP, allowing us to develop additional skills and to provide high levels of care within the community. Your role will allow you to rotate through specialist areas such as Hospital at Home, palliative care, old-age psychiatry and the community health and wellbeing hubs. This will provide you with the experience to work in our new frailty clinic where we provide rapid assessment, investigation and diagnostics in a setting that allows patients to remain at home and avoid unnecessary admission to an acute hospital. 


As we are a relatively small team there is scope to negotiate the structure of your working week, with a balance between GP sessions, frailty clinics and house visits, and education / professional development. There are also opportunities to be involved in service development.


Our current frailty clinics based in West Fife are closely affiliated with Medicine for the Elderly and are integrated with the multidisciplinary team at Whitefield Assessment and Rehabilitation Centre in Queen Margaret Hospital, Dunfermline. We have plans to expand this service into East Fife. Good administrative support has resulted in a robust working method for those who wish to work part time. We regularly liaise with our old-age psychiatry colleagues and use joint visits to improve service delivery for patients with complex physical and mental health needs.
