Reablement STARS – Specialist Therapist (Occupational Therapy/Physiotherapy)
Band 6 NHS (37.5 hrs) 

An opportunity has arisen for the role of Specialist Therapist within the integrated Reablement Service STARS working across Community Health and Social Care Partnership.

Supporting the delivery of the Recovery and Rehabilitation Framework can you inspire, enable and empower individuals, Carers and colleagues through compassionate conversations to work collaboratively to achieve what matters within improved health and wellbeing outcomes?  Can you demonstrate your skills through re-activation of daily living activities, self-management, support mental well-being and maximising independent living at home?  Do you invest time in enabling students and support an active learning environment?  This is an exciting opportunity to work in the Rehabilitation and Reablement Pathway as part of short term Reablement.

Can you confidently self-coordinate a complex caseload whilst proactively seeking shared best decisions with individuals and multi-disciplinary multi-agency partners?   Can you demonstrate effective working relationships across Locality Teams inclusive of GPs, Nursing, Social Work, Social Care, AHPs, Third and Independent sector organisations, Sport and leisure?  Do you enjoy fast paced, intensive treatment planning review and integrated collaborative working including close working with Paramedics through the SAS non-conveyance falls pathway?  Are you aware of Home Team and how you will be contributing towards working differently in the future to achieve transformational improvements for the health and well-being outcomes for the people of Dumfries and Galloway?

This post will primarily support needs within Annandale and Eskdale Locality however you will be expected to support regional Reablement demands.  

Please note – You must be able to travel independently.   A current driving licence is an essential requirement of this post. However, if you have a disability which precludes you from holding a driving licence consideration will be given if alternative transport can be arranged. Due to the ever changing health and social care climate, this post holder may be required in the future to work flexibly within the context of working regular weekends, to provide a 6 or 7 day per week service.  There is also a requirement to regularly flex to different localities if the need arises.  This will be discussed further at interview.
