An exciting opportunity has arisen through McMillan funding to provide effective programme management for implementation and ongoing development of Anticipatory Care Planning across the 6 Health & Social Care Partnerships (HSCPs) within NHS Greater Glasgow & Clyde.The job will also entail working jointly with 3'd and Independent sector organisations that deliver care alongside health and social work employees. The role will encompass the management of staff as well as close collaboration with Heads of Service, Service Managers and Senior Clinicians across HSCPS.

 You will facilitate consistent programme implementation and progression at an organisational level; supporting key stakeholders to fully implement ACPs and work closely with General Practitioners to encourage transcription of content to eKIS platform. You will be Educated to degree level or equivalent; with post-graduate professional education or have equivalent experience of programme management preferably within the Health and Social Care sector. Have effective communication and leadership skills with knowledge of change management and service re-design processes. A background in Health and Social Care is desirable, with knowledge of the National (iHub) ACP toolkit. This is a fixed term post for 22 months.
