An exciting opportunity has arisen through McMillan funding to support the implementation and ongoing development of Anticipatory Care Planning across the 6 Health & Social Care Partnerships (HSCPs) within NHS Greater Glasgow & Clyde. This job will also entail working jointly with the 3'd and Independent sector organisations that deliver care alongside health and social work employees .You will facilitate consistent programme implementation and progression at an organisational level; supporting local staff and ACP Champions to fully implement ACPs.
You will have awareness and an understanding of current clinical practices, strategic policies and guidelines. Fundamental to this, is expert knowledge and skill, partnership working and the maintaining of relationships and must be able to demonstrate excellent communication and leadership skills, work autonomously and deliver education sessions in a safe and effective way and facilitate the use of ACPs. You must be Educated to degree level or equivalent; with post-graduate professional education or have equivalent experience preferably within the Health and Social Care sector. This is a fixed term post for 22 months.
