To provide administrative and project support to the Programme Manager and Anticipatory Care Planning (ACP) Facilitators during the 2 year project to implement and develop Anticipatory Care Planning across the 6 Health & Social Care Partnerships (HSCPs) within NHS Greater Glasgow & Clyde. This includes supporting, facilitating and monitoring progress via data collection across the various work streams in each Partnership. To provide administrative support for the development and organisation of HSCP learning events. Supporting Programme Manager to produce monthly performance management information, updating project plans and collating reports on progress. You will be educated to Diploma level or possesses equivalent skills and competencies in typing and posses the European Computer Driving License or equivalent IT skills. You will have experience working within either NHS or Social Work and of working with data and information and have experience of organising learning events. The post is fixed term for 22 months.
