Pharmacists, are you ready for a change or to try something new? Wigtownshire Health and Social Care are looking to recruit a motivated, proactive and enthusiastic Pharmacist to work within the award winning Locality Pharmacy Team on a when required basis.
You will provide a clinical pharmacy service directly to patients and be involved in supporting the delivery of a pharmacotherapy service to the 9 GP Practices across the locality. The post holder will be based in GP practices and within the award winning pharmacy hubs. Day-to-day responsibilities might include: clinical assessment and review of patients, performing polypharmacy medication reviews with patients, dealing with medicines related queries, assessing patients ability to manage their medicines (including in the patient’s home), medicine reconciliation and prescribing of medication.
Joint working with other Community Health and Social Care colleagues will be routine, as well as with the multidisciplinary and professional teams within GP practices, Community Hospitals, Community Pharmacies and newly operational Home Teams.
