The post holder will have responsibility for caring for an individual patient in their home on a 2:1 basis.

 In partnership with the team, implement the plan of care to ensure delivery of a high level of care for the client who has autism spectrum disorder.

 Providing 2:1 support for the client in all aspects of activities of daily living.

 Works on a regular basis without direct supervision.

 As part of a multidisciplinary team, the post holder will carry out personal care duties for patients, in support of the registered nurse and other relevant professional

The post holder has delegated responsibility and accountability for the assessment and delivery of quality nursing care, primarily in the domiciliary setting to a practice population and the wider locality and in partnership with others. This includes monitoring and evaluating its effectiveness.

The postholder is responsible for raising awareness of health and social well being, influencing the broader context, enabling people to improve their own health and working collaboratively to tackle health inequalities. 

This post requires the post holder to have a PVG Scheme membership/ record. If the post holder is not a current PVG member for the required regulatory group (i.e. child and/or adult) then an application will need to be made to Disclosure Scotland and deemed satisfactory before they can begin in post.
