You will provide high quality, evidence based, culturally sensitive and effective specialist mental health nursing care and assessment to older adults experiencing mental health problems within a defined geographical area. You will act as a named nurse and be responsible for a caseload with regular caseload supervision provided by Band 6 CPN. You will be supported to engage in regular clinical supervision.
You will work as part of a multidisciplinary team within a CMHT, and contribute to toe the development and delivery of care provided and promote the role of the community mental health nurse. You will provide specialist advice, information, and education regarding nursing care to colleagues , patients and carers. You will provide triage, short term interventions and working in collaboration with colleagues to ensure effective, assessment, treatment and discharge planning, utilising a bio psychosocial model evidenced by high quality record keeping which reflects contemporaneous risk assessment and evidence based nursing care delivery . It is essential that you are currently a Registered Mental Heatlh Nurse with the NMC and that you have a completion of an evidence based psychological therapies/psychological approaches to care programme. The shift pattern for this post is Monday - Friday 9.00am - 5.00pm.
