This is a dynamic specialist Rehabilitation Service, within which the Support Worker is an integral member of the multidisciplinary team providing essential support to professional and senior staff. You will be delegated appropriate tasks by professionally qualified team member and will assist those professionals working under supervision and guidance in undertaking clinical and non-clinical tasks. You will be required to work independently in the Intermediate Care facilities and in a range of community and acute settings within the agreed rehabilitation/intervention plan. Intermediate The Intermediate Care Units enable rehabilitation and assessment to occur away from the pressures of an acute hospital, often when decisions regarding how patients’ needs are best met are being made. Assessment and interventions will also be carried out in patients' homes.

Experience of being part of a multi-disciplinary team in a rehabilitation, healthcare or similar setting is essential. It would be desirable if you have had experience of working in the community. A current Driving Licence is required for this post.

The shift pattern is weekdays and the occasional weekend.
