| Staff Nurse - Named Patient, Community, Complex Care |

Duties & responsibilities:
| We are now welcoming applications from Registered Learning Disability and Mental Health Nurses or Adult nurses with the relevant experience to a named patient care team, hosted by the Complex Care service in Midlothian. The post holder will have responsibility for caring for an individual client who has a learning disability and challenging behaviour in the client’s own home on a 2:1 basis. The post holder will be providing support for the client in all aspects of care and activities of daily living, ensuring delivery of a high level of care for the client. |

Further information: | Our aim is to provide high quality, consistent and continuous care and support enabling the patient to live in their own homes and as normal a life as possible. The team works in partnership with patients, families, Social Work, Education and respite facilities providing up to 24 hour, 7 days a week care through packages of care that are tailored to the individuals needs. |
