Enhanced Care Home Team – based within one of the three Locality Areas – Perth City, North Perth, South Perth


An exciting opportunity has arisen for an experienced individual to join the Enhanced Care Home Team within Perth and Kinross’ Health and Social Care Partnership. The successful candidate will be joining an integrated multi disciplinary team, and will be expected to demonstrate an ethos of whole system collaborative working and have highly developed skills to respond to the multiple and changing demands of the role, within set deadlines.  The post holder will be expected to liaise with all relevant partners and stakeholders and provide support, advice, guidance and leadership across the care home sector within Perth and Kinross.  In addition the post holder will require to work within a care home where it has been identified as meeting the exigencies of the service.

This is an excellent opportunity to join a newly established team with excellent training, supervision and development opportunities.   The successful candidate will be a registered nurse, Adult or Mental Health, and have a first level nursing qualification with first degree or other equivalent qualification;  appropriate part of NMC Register relevant to area; previous post registration experience within a relevant area and/or demonstrate expert clinical and professional practice developed through experience and theoretical knowledge; good  knowledge of relevant national ambitions and programmes, e.g. 2030 Vision;  evidence of  keeping up-to-date on all relevant professional issues with particular emphasis on new ways of working and innovation are some of the essential criteria for the post. Management experience or qualification relating to provision of education, practice development and/or quality improvement; experience or a working knowledge of the care home sector and an understanding of challenges facing Social Care across Perth and Kinross are all desirable.
