Applications are invited for individuals to join our Intensive Home Treatment Team in Perth.  The Team provides an emergency response, home treatment and early supported discharge service for people with mental health problems. 

As the successful applicant you will have experience of working as a Band 5 RMN with people who have a wide range of mental health problems.

As the post holder, you will provide specialist mental health interventions focusing on the management of crisis, home treatment and intensive support of people being discharged from in-patient care. You will work jointly with members of the Team to undertake individualised assessments of people in acute mental health crisis and will have a key role in developing, monitoring and evaluating treatment plans working in partnership with service users.

As a lone worker in the community you will develop, deliver and evaluate specialist psychological and psychosocial interventions working with people to facilitate, enable, and support them within their homes and communities.  You will also be required to provide a supervisory role to junior members of staff within the Team.

The ability to travel is essential to meet the job requirements, as is the ability to work as part of a shift rota system.
