NHS Orkney has an exciting opportunity for a Heart Failure Specialist Nurse to join our motivated and supportive Primary Care team, who is passionate about improving outcomes and experiences for heart failure patients.
As an experienced Heart Failure Nurse, you will establish a service which will aim to improve the management of patients with chronic heart failure, to reduce unnecessary hospital
re-admissions and to provide seamless care between primary, secondary and tertiary care.
You will work closely with both local multidisciplinary teams and our tertiary care Cardiology team in Grampian. In our remote and rural setting you must have the ability to work on your own initiative, tempered with the ability to recognise when referral on to a Cardiologist is required.
You will have the ability to provide expert information, education and support to heart failure patients and their families. You will work to develop local integrated care pathways which will help to ensure that local and national guidelines and targets are met, liaising closely with national and regional planning groups.
DEVELOPMENT OPPORTUNITY
The post may also be available as a trainee Heart Failure Nurse post, starting as a band 6, moving to a band 7 upon completion of required training and acquisition of relevant experience and qualifications. The minimum entry requirement for this programme is a degree or its equivalent in nursing or midwifery.
This post includes Distant Islands allowance, pro rata per annum
