You will be an experienced Health Care Support Worker who will support the District Nursing team in delivering care to patients within Renfrewshire, providing accessible, appropriate, high quality, culturally sensitive and effective nursing service to adults living in the community, by working in partnership with service users, care providers and other agencies.

To work collaboratively with the District Nursing team and assist in the implementation and evaluation of programmes of care for patients and their carers, under the supervision of the District Nurse or their deputy.

To contribute to health improvement initiatives to individuals and families on the caseload and to the wider community.

Required to work flexibly in terms of location to support other areas of team working within the DN service.

Recent experience working within a community setting is desirable.
