We are offering a unique opportunity to join a forward thinking, ambitious NHS Board run Practice in Angus where you can be part of exciting, new developments in General Practice
You will work with the in-house clinical team along with the Practice Development Manager supported by the Professional Clinical Development Lead GP for 2c Practices
You will be an integral part of the practice team, with the opportunity for close working with colleagues from both health and social care. This will include named consultant led teams in psychiatry, medicine for the elderly and palliative care working across the Locality. You will have the opportunity to lead on or contribute to the development of the range of services offered, whilst exploring new models of care to fit the needs of changing demographics
We will be looking to support new models of working, with opportunities for extended appointment times, on the day access, and close links with community resources including community pharmacy, community nursing and local voluntary sector. We have Weekly MDT meetings with community nursing, medicine for the elderly team, community pharmacist and social care teams to help pro-actively plan and manage care for frail elderly patients
