​​​​​​​​​​​​​​We have an exciting opportunity for an Advanced Nurse Practitioner to join our dynamic team of advanced nurse practitioners within NHSL Lanarkshire. We are looking for ANP qualified staff who are highly motivated and enjoy a challenge. We would also be happy to consider annex 21.

The Advanced Practitioners (Nurse, Community) will work autonomously across NHS Lanarkshire within Primary, Community Care during the in and out of hours period, covering seven days including public holidays.  They will be responsible for the clinical decision making for the population including assessment and treatment of patients in across the primary and community settings.

The Advanced Practitioners (Nurse, Community) will work closely with GP Practices including the OOH Hub and communicate on a daily basis about any patient they have seen overnight/ at weekend and who needs to be followed up by the GP during normal working hours.

The Advanced Practitioner (Nurse, Community)  provides assessment,  clinical care and management of both stable and acutely unwell  patient and his/her family, making complex, autonomous decisions using advanced skills and knowledge, having overall responsibility for the assessment of care needs and the delivery of advanced nursing care.   Medical telehealth/telephone support will be available when required from an identified source. ​​​​​​​
