We have an exciting opportunity for an experienced manager to join Perth & Kinross HSCP Primary Care team, to lead and facilitate key aspects of implementation of primary care improvement and development, including the next phase of the Primary Care Improvement Plan. In addition to this central role, you will manage the development and day-to-day function of the newly established multidisciplinary Primary Care Resilience Team, which will provide essential support to G.P. practices when required.
We are looking for an individual with experience of managing and leading change in a health setting to support Primary Care in Perth & Kinross. You will work closely and effectively with general practice teams, multidisciplinary health and social care teams and the wider community to develop new primary care models, which ensure we deliver effective and efficient services which support HSCP strategic plans.
You will have a track record in delivering successful projects and supporting organisational, sustaining relationships in order to achieve results.
Communication skills, written and oral, must be highly developed.
You will be required to achieve the mandatory Induction standards for Healthcare Support Workers in Scotland and to comply with the Code of Conduct throughout your employment.

Hours of work are: 37.5 hours per week
Fixed Term/Secondment for 12 months
