The Sources of Support social prescribing service is a partnership between Health Inequalities Service, the Scottish Government, and Dundee Health and Social Care Partnership. The service was expanded through funding initially through the Scottish Government Community Link Workers Programme, the focus of which was to reduce health inequalities and facilitate access to non-clinical support and interventions. Subsequently, this work was incorporated into the Memorandum of Understanding as one of the national priorities for development through the Primary Care Improvement Plan and Action 15 developments within mental health. Sources of Support sits within Dundee’s integrated Health Inequalities Service. 

An exciting opportunity has arisen for an experienced candidate to take on the role of    link worker in identified GP Practices in Dundee.  

The link worker will provide person-centred support to referred patients to help them identify priorities and outcomes and access a wide range of non-clinical services and activities to improve their health, wellbeing and life circumstances.  

Candidates must be autonomous practitioners with experience of 1 to 1 case management with people who have complex social and emotional circumstances; a strong understanding of the challenges faced by people living in deprivation; community based, person-centred, non-clinical approaches; and collaborative, multi-disciplinary working. Applicants must be educated to degree level in a relevant area, preferably with evidence of post-graduate training or qualifications. It is desirable that applicants have previous experience of working within social prescribing services.

You will be required to achieve the mandatory Induction standards for Healthcare Support Workers in Scotland and to comply with the Code of Conduct throughout your employment.
