Exciting opportunities for experienced Physiotherapists, Occupational Therapists and Health Care Support Workers to join our expanding Intermediate Care Teams as part of Fife’s Integrated Community & Support Services (ICASS).

Health and Social Care Partnership are enhancing their community and inpatient therapy services, to ensure high standards of rehabilitation from robust and responsive teams over a 7 day period.

The Intermediate Care Service provides rehabilitation, which is essential in maintaining patient flow through the whole system, to the adult population across a variety of settings.  Various positions are available across Fife.  Within inpatients (including stroke rehabilitation), community rehabilitation and mixed posts that include both inpatient and community working.

The primary function of these roles, will be to work within the multidisciplinary Intermediate Care Teams to assess and deliver therapeutic treatment programmes to service users.  These services improve patient flow within inpatient settings, supporting timely discharge from hospitals, and prevent hospital admissions from the community.

Professional staff will require to be a qualified Physiotherapist or Occupational Therapist and be registered with the HCPC.  Support workers will be required to have an SVQ Level III in Health and Social Care or evidence of equivalent, and experience working as a support worker would be desirable. Relevant training and competencies will be supported on commencing the post.

Some of the support worker posts will be based with the function of daily rehabilitation in the community; a service provided 365 days of the year from 0800-2200 and will therefore operate over shifts to cover these hours.

Access to the community services over 7 days are now being developed, and weekend working will be expected on a rota basis for registered staff.

Experience of working with older people is essential as they are the main group of service users.  Experience in working with people with stroke, dementia, Parkinson’s disease and general frailty would be desirable.

You will have excellent organisational and communication skills and be able to work independently as well as within the team.  There may be occasions where you will be required to work in one of the other community teams or hospital sites as we work flexibly to meet demand.

Our service offers regular supervision, PDP, supports training opportunities, and regularly supports students undertaking clinical placements.  Our diverse service also has link worker roles in each of the teams to support stroke, dementia and major trauma work.  Rehabilitation beds are available in care home settings known as STAR beds (short term assessment and rehabilitation) and our services provide therapy input to those units.

The ability to travel throughout the area is essential.
