A new role created within NHS Fife offers an opportunity for a pharmacist to work in a clinical liaison role as part of a multi-agency Reach, Engage, Act in Communities and Hospital team (REACH) to promote patient engagement with services both during acute hospital admission and post-discharge. Working closely with the Specialist Pharmacist in Substance Misuse (SPiSM) you will develop the pharmaceutical care component of this new service to ensure that patients receive maximum benefit from the management of the substance misuse aspects of their care.

You will have a pivotal role in the REACH Team to ensure that patients get a full assessment, review and management of their prescribed medicines and illicit drug use.  You will have a key role in liaising with in-patient and primary care based services which will include the acute pharmacy team and community pharmacy harm prevention services as part of the post-discharge planning process. You will possess excellent communication, organisational and negotiation skills and have the ability to take forward service development, redesign and change management.

You will be involved in education and training of other professionals and patients and be expected to provide support and deputise for senior pharmacists and undertake supervision of less experienced pharmacists, technicians and students. You should have a sound clinical knowledge and a commitment to high standards of pharmaceutical care. It would be desirable though not essential to have previous experience of working within a Substance Misuse environment.

You will have a Masters degree in Pharmacy or equivalent and be on the practicing register of the General Pharmaceutical Council. You will have demonstrable clinical pharmacy experience and will have completed the Scottish Hospital Vocational Training scheme or be able to demonstrate that you have equivalent experience. A Clinical Pharmacy diploma/MSc and/or independent prescribing qualification are desirable or a willingness to undertake the same.

This is an ideal opportunity to gain specialist clinical pharmacy expertise within a richly rewarding clinical area. You should be flexible and be willing to travel throughout Fife.
